Frequently Asked QuestionsQ. What is the Health & Wealth Raffle?A. The Health & Wealth Raffle is a fund raising event designed to generate substantial new revenues for St. Joseph's Hospital and Medical Center. It is not considered philanthropy in the traditional sense. Instead, the Raffle is a gaming event, using the entertainment and top-dollar prize format to attract dollars in support of the hospital. The revenue generated for St. Joseph's Hospital and Medical Center and its Barrow Neurological Institute by the Raffle supports patient care, education and medical research. Arizona's Health & Wealth Raffle is the largest of its kind in the United States. Back to top>Q. Where does the money go?A. The Health & Wealth Raffle contributes to the community in three major ways. 1) It provides new revenues for programs and services at St. Joseph's and Barrow. 2) It generates a sizeable economic impact for Arizona – each Raffle returns approximately $6 million to $8 million to businesses in the community. 3) Thousands of winners in the Raffle will enjoy prizes valued from $100 to more than $1,000,000. Back to top>Q. How many prizes are there?A. There are 7,400 individual prizes to be won in the main Raffle. The Early Bird Draw offers two prize packages. The Final Draw will consist of 7,398 prizes, including the three grand prizes, cars, trips, electronics, golf prizes, jewelry and around-the-home prizes. Back to top>Q. What are the odds of winning a prize?A. The odds of winning a prize are 1-in-18 with the purchase of one ticket, a number verified by the accounting firm CBIZ Accounting, Tax & Advisory Services, LLC. Holding multiple tickets does not change the odds that any one ticket will win, rather it gives a person more chances to win – each of their tickets having 1-in-18 odds of winning a prize. Back to top>Q. How are the prizes drawn?A. The draws are conducted electronically by CBIZ Accounting, Tax & Advisory Services, LLC. In each draw, the prizes with the lowest manufacturer's suggested retail price will be drawn first, progressing in order of value to the top Grand Prize. The algorithm and random number generator used to conduct the draw and their ability to generate truly random numbers have been tested and confirmed by an independent third-party specializing in the testing of gaming devices. Back to top>Q. If someone wins a prize, is that original entry still eligible for additional prizes?A. Yes. All ticket numbers are eligible for all prizes. After each draw, the winner's Raffle number will be returned to the draw, and each entry remains in the draw for all subsequent prize drawings. It is possible that a person who purchases only one ticket could win more than one prize. Back to top>Q. How much does it cost to participate?A. Single tickets are $100, and a three-pack of tickets can be purchased for $250. Individuals may enter as many times as they wish. Back to top>Q. How many tickets are available?A. Raffle entries will be for sale to the general public beginning Wednesday, August 6, 2008. Entries will be accepted by telephone toll-free at 1-866-390-9034, in person at the main lobby of St. Joseph's Hospital and via the Internet at www.HealthWealthRaffle.org. Back to top>Q. Will I receive a ticket?A. No. Tickets are in electronic form; there are no hard copy tickets. You will receive a receipt that indicates your ticket numbers, either online, if we have your email address, or through the mail. Please refer to the Raffle rules and regulations for details. Back to top>Q. When does the Raffle take place?A. The Fall 2008 Health & Wealth Raffle celebrates its public launch on Wednesday, August 6, 2008. An Early Bird Draw for people who have purchased a Raffle entry by midnight, Wednesday, September 17, 2008, will be held on Friday, September 26, 2008. The Final Draw for the remaining prizes will occur on Thursday, October 9, 2008, or sooner in the event of early sell out. The entry deadline for the Final Draw is midnight, Wednesday, October 1, 2008. In the event all entries are sold by the Early Bird deadline on Wednesday, September 17, 2008, all 7,400 prizes will be drawn on Friday, September 26, 2008. If ticket sellout is not achieved by the Early Bird cutoff of September 17, but occurs prior to the Final Draw entry deadline of October 1, the Final Draw for 7,398 prizes will be held ten days after sellout or on the scheduled draw date, whichever comes first. If the tenth day falls on a weekend or legal holiday, the draw will be held the next business day. Back to top>Q. Who is eligible to purchase Raffle entries?A. In order to participate in the Heath & Wealth Raffle, the purchaser must be at least 21 years of age and present in the state of Arizona at the time of purchase. Anyone 21 years of age or older and in Arizona can purchase a ticket for someone else (whether that person is in Arizona or not) – the purchaser simply buys the ticket(s) and transfers the ticket(s) to whomever they wish as long as the transferee is also 21 years of age or older. Board members and executive management staff of St. Joseph's Hospital and Medical Center, St. Joseph's Foundation and Barrow Neurological Foundation and employees of CBIZ Accounting, Tax & Advisory Services, LLC and CBIZ, Inc., and family members living in the same household as all of the above are not eligible to purchase an entry or win a prize. Back to top>Q. How will the winners be notified?A. CBIZ Accounting, Tax & Advisory Services, LLC will notify all winners by courier, mail or email of what they have won, how to claim their prize and the time frame in which to do so. Grand Prize winners may be notified by telephone. Prize suppliers may also contact winners via the telephone and/or email to verify the shipping address for prizes. Back to top>Q. What happens if a winning ticket has more than one name listed?A. In the event a winning entry has more than one name listed on the ticket, the Administrator, CBIZ Accounting, Tax & Advisory Services, LLC, will physically award the prize to the first name listed on the entry. For tax purposes, the prize can be split among all ticket purchasers. CBIZ Accounting, Tax & Advisory Services, LLC, Black Fund Development, Inc., St. Joseph's Hospital and Medical Center, St. Joseph's Foundation and Barrow Neurological Foundation are not responsible for resolving disputes which may arise from any conflict between joint ticket purchasers. Back to top>Q. What does a prizewinner need to claim his/her prize?A. Some winners may be required to present their award letter and two forms of identification that verify their name, address and social security number or individual taxpayer identification number. Examples of acceptable identification include Social Security Card, Voter Identification Card, valid Driver's License, Passport or ID card issued by a federal, state or local government. Winners do not need their entry verification or ticket to claim their prizes. Prizes may be transferred to another winner by providing proper documentation to the Administrator and prize supplier. Back to top>Q. What else do players need to know?A. The Health & Wealth Raffle is a form of gaming and according to IRS ruling, the cost of the entry does not qualify as a charitable deduction although it may qualify as a gambling deduction. Please consult your tax advisor for specific applicable tax rules. The IRS further requires corresponding income tax withholding on prize values of $5,000 or greater must be paid prior to prize distribution.
