FREQUENTLY ASKED QUESTIONS
Q. What is St. Joseph's Health & Wealth Raffle?
A. St. Joseph’s Health & Wealth Raffle is an opportunity for the public to participate in generating new revenue for St. Joseph's Hospital and its Barrow Neurological Institute by purchasing tickets that could win a prize. Net proceeds from the Raffle go toward patient care, medical education and/or research – in keeping with Arizona legislation authorizing such a Raffle.
You may have heard of St. Joseph’s Health & Wealth Raffle called "Arizona’s Original Raffle." That is because St. Joseph’s began this fund raising concept in Arizona in 2003, and it is now the longest established of its kind in the United States. St. Joseph’s Health & Wealth Raffle is proud of this fact – and even prouder of the amazing things happening every day at St. Joseph’s Hospital and Barrow Neurological Institute as a result of the dollars generated by this project. St. Joseph’s Health & Wealth Raffle is not philanthropy in the traditional sense. Instead, it uses an entertainment and prize format to attract dollars in support of the hospital, as a gaming event would.
Q. Where does the money go?
A. St. Joseph's Health & Wealth Raffle contributes to the community in three major ways: 1) providing new revenues for programs and services at St. Joseph's and Barrow, 2) generating a positive economic impact for Arizona – each Raffle returns more than $5 million to businesses in the community, and 3) the enjoyment of the many Raffle winners, with prize values between $100 – $1,000,000.
Q. How many prizes are there?
A. There are 4,015 individual prizes to be won in the raffle, with the Appreciation Draw offering 10 prize packages and the Early Bird Draw offering two (2) Early Bird prize packages. The Final Draw consists of 4,003 prizes, including two (2) grand prizes plus cars, trips, entertainment, electronics, jewelry and around-the-home prizes.
Q. What are the odds of winning a prize?
A. The odds of winning a prize are at least 1-in-18 with the purchase of one ticket, as verified by the accounting firm CBIZ MHM, LLC. Holding multiple tickets does not change the odds that any one ticket will win; rather it gives a person more chances to win – each of their tickets having 1-in-18 odds of winning a prize.
Q. How are the prizes drawn?
A. The draws are conducted electronically by CBIZ MHM, LLC. In each draw, the prizes with the lowest manufacturer's suggested retail price will be drawn first, progressing in order of value to the Grand Prizes. The algorithm and random number generator used to conduct the draw and their ability to generate truly random numbers have been tested against industry standards for confirming random number generation and found to comply with such standards.
Q. If someone wins a prize, is that original entry still eligible for additional prizes?
A. Yes. All ticket numbers are eligible for all remaining prizes. After each draw, the winner's raffle number will be returned to the draw, and each entry remains in the draw for all subsequent prize drawings. It is possible that a person who purchases only one ticket could win more than one prize.
Q. Can I have my prize shipped out-of-state, and can I get a substitution or cash instead of the prize I've won?
A. With the exception of vehicles and real estate, prizes can be shipped out-of-state. Unless specified in your prize letter, there are no prize substitutions; and, as stated in the Rules & Regulations, only specified prizes may be exchanged for cash value.
Q. How much does it cost to participate?
A. Single tickets are $100 and a three-pack of tickets can be purchased for $250. Individuals may enter as many times as they wish as long as tickets are available.
Q. How many tickets are available?
A. A total of 67,900 tickets will be available in the Spring 2012 Raffle, with a limited quantity of 3-packs.
Q. How does someone enter the raffle?

Raffle entries will be for sale to the general public beginning Friday, February 24, 2012. Entries are accepted: 1) by telephone, toll-free at 1-866-390-9034, 2) in person at the main lobby of St. Joseph's Hospital, and 3) via the Internet at www.healthwealthraffle.org.
Q. Will I receive a ticket?

If all entries are sold by the Early Bird deadline at midnight on Thursday, March 15, 2012, all remaining 4,005 prizes will be drawn on Thursday, March 22, 2012. If ticket sellout is not achieved by the Early Bird cutoff of March 15, 2012 but occurs prior to the Final Draw entry deadline of April 5, 2012, the Final Draw for 4,003 prizes will be held ten days after sellout or on the scheduled draw date, whichever comes first. If the tenth day falls on a weekend or legal holiday, the draw will be held the next business day.
Q. Who is eligible to purchase raffle entries?
A. In order to participate in St Joseph's Heath & Wealth Raffle, the purchaser must be at least 21 years old and be present in the state of Arizona at the time of purchase. Entry purchaser, and anyone to whom an entry purchaser legally transfers an entry, must be either a business entity or an individual 21 years or older at time of entry purchase. Individuals buying on behalf of a business/legal entity must be legal representatives of the business and must, themselves, be 21 years or older at the time of entry purchase. Anyone 21 years old and in Arizona can purchase a ticket for someone else (whether that person is in Arizona or not) – the purchaser simply buys the ticket(s) and transfers the ticket(s) to whomever they wish as long as the transferee is also 21 years of age or older. Board members and executive management staff of St. Joseph's Hospital and Medical Center, St. Joseph's Foundation and Barrow Neurological Foundation, employees of CBIZ MHM, LLC and CBIZ, Inc., and family members living in the same household as all of the above are not eligible to win a prize.
Q. How will the winners be notified?
A. CBIZ MHM, LLC will notify all winners by courier, mail or email of what they have won, how to claim their prize and the time frame in which to do so. Winners may also be notified by telephone. Prize suppliers may also contact winners via the telephone and/or email to verify the shipping address for prizes.
Q. What happens if a winning ticket has more than one name listed?
A. As stated in the Rules & Regulations, the prize will be delivered to the first name on the entry.
Q. What does a prize winner need to claim his/her prize?
A. Some winners may be required to present their award letter and two forms of identification that verify their name, address and social security number or individual taxpayer identification number. Examples of acceptable identification include Social Security Card, Voter Identification Card, valid Driver's License, Passport or ID card issued by a federal, state or local government. Winners do not need their entry verification or ticket to claim their prizes. Prizes may be transferred to another winner by providing proper documentation to the Administrator and prize supplier.
Q. What else do players need to know?
A. Even though the proceeds benefit St. Joseph's Hospital and its Barrow Neurological Institute, the cost of the entry does not qualify as a charitable deduction since it is deemed to be a form of gaming. An entry may qualify as a gambling deduction; however, please consult your tax advisor for specific applicable tax rules. The IRS requires income tax withholding on prize values of $5,000 or greater, which must be paid prior to prize distribution. Prizes must be accepted as awarded with no cash-in-lieu-of option, except where otherwise indicated in the brochure.
