Q. What is the Health & Wealth Raffle, benefiting Barrow & St. Joseph's?
A. The Health & Wealth Raffle, benefiting Barrow & St. Joseph's, is an opportunity for the public to participate in generating new revenue for Barrow Neurological Institute and St. Joseph's Hospital and Medical Center by purchasing tickets that could win a prize. Net proceeds from the Raffle go toward patient care, medical education and/or research – in keeping with Arizona legislation authorizing such a Raffle.
You may have heard the Health & Wealth Raffle called "Arizona’s Original Raffle." That is because Barrow and St. Joseph’s began this fundraising concept in Arizona in 2003, and it is now the longest established fundraiser of its kind in the United States. The Health & Wealth Raffle is proud of this fact – and even prouder of the amazing things happening every day at Barrow Neurological Institute and St. Joseph's Hospital as a result of the dollars generated by this project. The Health & Wealth Raffle is not philanthropy in the traditional sense. Instead, it uses an entertainment and prize format to attract dollars in support of Barrow and St. Joseph's.
Q. Where does the money go?
A. The Health & Wealth Raffle contributes to the community in three major ways: 1) It provides new revenues for research, medical education and patient care at Barrow and St. Joseph’s. 2) It generates a positive economic impact for Arizona – the majority of services and prizes (valued at more that $2 million) are purchased in Arizona. 3) Raffle winners come away with valuable and exciting prizes. To date, more than two million prizes have been awarded by the Raffle.
Now celebrating its 10th anniversary, the Health & Wealth Raffle has sponsored 18 successful raffles and contributed more than $50 million to research, medical education and patient care at Barrow and St. Joseph's.
Q. How many prizes are there?
A. The Spring 2013 Health & Wealth Raffle will give away 2,026 prizes in the Final Grand Prize Draw, including three grand prizes plus cars, trips, entertainment, electronics, jewelry and cash. Plus, there are 11 prize packages in the five Early Draws.
Q. What are the odds of winning a prize?
A. The odds of winning a prize are at least 1-in-40 with the purchase of one ticket, as verified by the accounting firm CBIZ MHM, LLC. Holding multiple tickets does not change the odds that any one ticket will win; rather it gives a person more chances to win since each ticket has a 1-in-40 chance of winning a prize.
Q. How are the prizes drawn?
A. The draws are conducted electronically by CBIZ MHM, LLC. In each draw, the prizes with the lowest manufacturer's suggested retail price will be drawn first, progressing in order of value to the Grand Prizes. The algorithm and random number generator used to conduct the draw and their ability to generate truly random numbers have been tested against industry standards for confirming random number generation and found to comply with such standards.
Q. If someone wins a prize, is that original entry still eligible for additional prizes?
A. Yes. All ticket numbers are eligible for all remaining prizes. After each draw, the winner's ticket number will be returned to the draw, and each entry remains in the draw for all subsequent prize drawings. It is possible that a person who purchases only one ticket could win more than one prize.
Q. Can I have my prize shipped out of state, and can I get a substitution or cash instead of the prize I've won?
A. With the exception of vehicles and real estate, prizes can be shipped out of state. Unless specified in your prize letter, there are no prize substitutions, and, as stated in the Rules & Regulations, only specified prizes may be exchanged for cash value.
Q. When does the raffle take place?
A. The Appreciation Phase of the Spring 2013 Health & Wealth Raffle begins on Wednesday, February 13, 2013. Draw dates are: Appreciation Draw–March 12, 2013, for which entries will be accepted until midnight March 5, 2013; Early Draw #1–March 26, 2013, for which entries will be accepted until midnight March 21, 2013; Early Draw #2–April 2, 2013, for which entries will be accepted until midnight March 28, 2013; Early Draw #3–April 9, 2013, for which entries will be accepted until midnight April 4, 2013; Early Draw #4–April 16, 2013, for which entries will be accepted until midnight April 11, 2013: Final Grand Prize Draw–April 25, 2013, for which entries will be accepted until midnight April 18, 2013. Deadlines will be adjusted in the event of early sellout as described in the following paragraph.
Should all entries be sold out by midnight of any early draw date, all draws for the remaining prizes will be held on the 7th calendar day following the sell out date. Entry purchasers may win more than one prize. After each draw, the winner’s entry number will remain eligible for all subsequent draws. The first draw for which an entry is eligible is determined by the entry deadline, and the entry remains eligible for that first draw and all subsequent prize drawings. If entries are not sold out by midnight of any early draw entry deadline, entry purchasers who purchased before or after midnight of each early draw entry deadline are eligible for all of the remaining 2,026 prizes on the Grand Prize Draw date of April 25, 2013. In the event entry sellout occurs after midnight of any early draw entry deadline, but before midnight April 18, 2013, the Grand Prize Draw will be held on the 7th calendar day following the sell out date. If the 7th calendar day falls on a weekend or legal federal or state holiday, the draw will be held on the next business day.
Q. How many draws are there?
A. In this spring’s Raffle, there is a total of six draws. This includes the following drawings: Appreciation Draw, four Early Draws, and the Final Grand Prize Draw.
Q. What is the Appreciation Draw? How is someone eligible to enter?
A. The appreciation phase is a special opportunity for previous Health & Wealth Raffle participants to buy their raffle tickets before the Raffle is advertised to the general public. These early ticket buyers are entered into the Appreciation Draw.
Q. How much does it cost to participate?
A. Single tickets are $100, and a three-pack of tickets can be purchased for $250. Individuals may enter as many times as they wish as long as tickets are available.
Q. How many tickets are available?
A. A total of 80,000 tickets will be available in the Spring 2013 Raffle, with a limited quantity of three-packs.
Q. How does someone enter the raffle?
A. Raffle entries will go on sale beginning February 13, 2013, with advertising directed toward previous players only. (Appreciation Phase) Raffle entries will be advertised for sale to the general public beginning Wednesday, March 6, 2013. Entries are accepted: 1) by telephone, toll-free at 1-866-658-4065, 2) in person at the main lobby of St. Joseph's Hospital, and 3) via the Internet at www.HealthWealthRaffle.org.
Q. Will I receive a ticket?
A. No. Tickets are in electronic form; there are no hard copy tickets. You will receive a receipt that indicates your ticket numbers either online, if we have your email address, or through the mail. Please refer to the Raffle rules and regulations for details.
Q. If I am a Grand Prize winner, will my name be publicized?
A. Yes. By purchasing an entry in the Raffle, you give permission for the Raffle, Barrow Neurological Foundation, St. Joseph’s Foundation, Barrow Neurological Institute and St. Joseph’s Hospital and Medical Center to publish your name, location (city and state) and the prize you won. In addition, a Grand Prize Winner will be required to execute and return an Affidavit of Eligibility, a Liability Release and a Publicity Release before receiving any prizes.
Q. Who is eligible to purchase raffle entries?
A. In order to participate in the Heath & Wealth Raffle, entry purchaser, and anyone to whom an entry purchaser legally transfers an entry, must be either a business entity or an individual 21 years or older at time of entry purchase. Individuals buying on behalf of a business/legal entity must be legal representatives of the business and must, themselves, be 21 years or older at the time of entry purchase. It is not necessary to be a resident of the state of Arizona, but one must be physically present in Arizona at the time of entry purchase. Only one (1) individual may be identified as an entry purchaser. By entering the Raffle, the entry purchaser acknowledges that he or she is aware of and agrees to abide by the Raffle’s rules and regulations. The following individuals and their respective affiliates, subsidiaries, parent and related companies and agencies are not eligible to win a prize in the Raffle: (a) Board members and Executive Management Team of St. Joseph’s Hospital and Medical Center; (b) Dignity Health Arizona Service Area Leadership Team (c) Board members and Executive Management staff of St. Joseph's Foundation and Barrow Neurological Foundation (including Directors, Vice Presidents and CEO) (d) the employees of CBIZ, Inc. and its subsidiaries. The above list of exclusions (items a, b, c, d) also applies to immediate family members residing in the same household.
Q. How will the winners be notified?
A. CBIZ MHM, LLC will notify all winners by courier, mail or email of what they have won, how to claim their prize and the time frame in which to do so. Winners may also be notified by telephone. Prize suppliers may also contact winners via telephone and/or email to verify the shipping address for prizes.
Q. What happens if a winning ticket has more than one name listed?
A. As stated in the Rules & Regulations, the prize will be delivered to the first name on the entry.
Q. What does a prize winner need to claim his/her prize?
A. Some winners may be required to present their award letter and two forms of identification that verify their name, address and social security number or individual taxpayer identification number. Examples of acceptable identification include Social Security card, voter identification card, valid driver's license, passport or ID card issued by a federal, state or local government. Winners do not need their entry verification or ticket to claim their prizes. Prizes may be transferred to another winner by providing proper documentation to the Administrator and prize supplier.
Q. What else do players need to know?
A. Even though the proceeds benefit Barrow Neurological Institute and St. Joseph’s Hospital, the cost of the entry does not qualify as a charitable deduction since it is deemed to be a form of gaming. An entry may qualify as a gambling deduction; however, please consult your tax advisor for specific applicable tax rules. All winners are responsible for any and all federal, state and local income or excise taxes, fees, assessments and like charges associated with the prizes. The IRS requires income-tax withholding on prize values of $5,000 or greater, and this tax must be collected prior to prize distribution. All cash prizes will be paid net of applicable income tax withholding. Prizes must be accepted as awarded with no cash-in-lieu-of option, except where otherwise indicated on the website, www.HealthWealthRaffle.org.

ELIGIBILITY Original entries are entries purchased directly from the Health & Wealth Raffle, benefitting Barrow Neurological Foundation and St. Joseph’s Foundation (the “Raffle”). Original entry purchaser, and anyone to whom an original entry purchaser legally transfers an entry, must be either a business entity or an individual twenty-one (21) years of age or older at time of entry purchase. Individuals purchasing an entry on behalf of a business/legal entity must be legal representatives of the business/legal entity and must, themselves, be twenty-one (21) years of age or older at the time of entry purchase. Original entry purchaser must be present in the state of Arizona at the time of entry purchase. For all other entry purchasers, ticket confirmation number(s) will be mailed within ten (10) business days of purchase. A hard copy ticket confirmation number is not required to claim a prize. Questions concerning ticket confirmation number(s) should be directed to the “Administrator,” CBIZ MHM, LLC, at the address listed below. For security purposes, the Administrator will attempt to confirm all purchases of $2,000.00 or more. Orders that cannot be confirmed will not be filled. By entering the Raffle, the entry purchaser acknowledges that he or she is aware of, and agrees to abide by, the Raffle’s rules and regulations. Entries received after the requested ticket package is sold out will be returned to the requesting entrant. Requests for ticket cancellations and ticket reassignments must be made by noon of the day that is at least three (3) business days prior to the date of the first draw for which the entry is eligible. All entries are non-refundable after the applicable noon deadline or once an entry has been included in any draw. In the Raffle, there is no possibility of a tie. All draws will be under the supervision of the Administrator. All time references to noon and midnight are Mountain Standard Time.
PRIZES 2,026: ODDS 1/40 Prizes will be awarded in the Grand Prize Draw, April 25, 2013. Prizes may not be exactly as shown in the brochure, in television commercials, or on the web site and are based on prize suppliers’ availability. If unavailable, similar or like kind prizes may be substituted at the sole discretion of Barrow Neurological Foundation Director of the Raffle. The approximate retail value of all combined prizes is more than $2,000,000 MSRP (manufacturer’s suggested retail price), based on pricing at the time of prize selection. All MSRPs are rounded to the nearest dollar. Only specified prizes may be exchanged for cash value. When airfare is included in a prize, unless otherwise stated, the fare is round trip coach class from Phoenix, Arizona, for two persons. With proper transfer or sales documents bearing notarized signatures, as required by the Raffle, a prize may be transferred or sold. Prizes may also be donated back to Barrow Neurological Foundation to benefit Barrow Neurological Institute and St. Joseph’s Hospital and Medical Center.
NOTIFICATION OF WINNERS All winners will be notified via courier, mail or by email (where available) by the Administrator of the requisite procedure to claim the prize. All Grand Prize winners must claim their prize within fourteen (14) calendar days of the draw. Winners of vehicles are required to claim their prize within twenty-one (21) calendar days of the draw (with the exception of custom vehicles, details for which will be provided in winner notification). All vehicle winners shall be responsible for any and all state or local license, title, registration, documentation and prep fees associated with the vehicles. Vacation packages must be claimed within forty-five (45) calendar days of the draw. All remaining prizes are to be claimed by winners within three (3) months of the draw unless otherwise stipulated in the prize letter. Three (3) business days after the completion of the Grand Prize Draw, entry purchasers can access www.healthwealthraffle.org to check prize winning status using their ticket confirmation number. Also at that time, a complete list of the names of prize winners and locations (by city or town and state) will be available on the website. A printed version of the list of prize winners will be available within thirty (30) days of the Grand Prize Draw by contacting the Administrator (contact information below). If a selected winner is unable to be contacted or located, using the information provided with the entry, prior to the deadline for claiming the prize, that prize or a cash equivalent will revert to Barrow Neurological Foundation in support of Barrow Neurological Institute and St. Joseph’s Hospital and Medical Center. In the event a winning entry has more than one name listed on the ticket, the Administrator, CBIZ MHM, LLC, will physically award the prize to the first name listed on the entry. For tax purposes, the prize can be split among all prize winners. Neither CBIZ MHM, LLC, Health & Wealth Raffle, benefitting Barrow and St. Joseph’s Barrow Neurological Institute, St. Joseph’s Hospital and Medical Center, St. Joseph’s Foundation and Barrow Neurological Foundation nor their legal counsel are responsible for resolving disputes which may arise from any conflict between or amongst joint ticket holders or purchasers.
PRIVACY By entering the Raffle, entry purchasers agree that their contact information provided in connection with the Raffle entry may be used for communications promoting the Raffle and future raffles sponsored by or conducted for the benefit of Barrow Neurological Institute and St. Joseph’s Hospital and Medical Center. An entry purchaser may subsequently “opt-out” of the receipt of such communications by following the directions in any e-mail, other communication or by contacting the Administrator (contact information below). Other than as provided herein, an entry purchaser’s information will not be shared with or sold to any other person or entity. Purchase of an entry in the Raffle, constitutes permission for the Raffle, Barrow Neurological Foundation, St. Joseph’s Foundation, Barrow Neurological Institute and St. Joseph’s Hospital and Medical Center to publish winner’s name, location (city and state) and prize won for purposes of advertising the Raffle or future raffles without further consent or compensation, unless prohibited by law.
REQUIREMENTS Winners may be required to execute and return an Affidavit of Eligibility, a Liability Release and a Publicity Release (the “Affidavit/Release”). The Affidavit/Release must be returned by the deadline stated in such document. Until the requested Affidavit/Release is returned, the winner is only a presumptive winner. If the presumptive winner fails to return such documents by the return deadline, or if an entrant is found to be ineligible, or if a presumptive winner does not comply with the Official Rules, then the presumptive winner will be disqualified and the prize or a cash equivalent will revert to Barrow Neurological Foundation.
WAIVER / LIMITATIONS OF LIABILITY Entry purchasers, by participating (and winners, by accepting a prize), release Dignity Health d/b/a St. Joseph’s Hospital and Medical Center, Barrow Neurological Institute, St. Joseph’s Foundation and Barrow Neurological Foundation, and their respective parent companies, affiliates, partners, subsidiaries, agents, representatives, sponsors, service agencies and independent contractors, and each of their respective directors, officers, partners, employees and agents, including advertising and promotion agencies, from any and all liability with respect to participation in the promotion, possession, use and distribution of the prize(s) awarded.
INTERNET The Raffle will pursue all available legal remedies against any individual(s) damaging or attempting to damage any Health & Wealth Raffle, benefitting Barrow and St. Joseph’s website or attempting to undermine the lawful operation of the Raffle.

TAXES AND IRS RULES All winners shall be responsible for any and all federal, state and local income or excise taxes, fees, assessments and like charges associated with the prizes. All cash prizes will be paid net of applicable income tax withholding.
SPECIAL NOTE: At the time of this printing, federal law requires that corresponding tax withholding on prize values of $5,000 or greater must be collected from winners upon award and distribution.
